Apply for ST / Caste Certificate?

Application forms for Scheduled Tribe and Scheduled Caste Certificates are available at Certificate Branch, DC Office. Form can also be downloaded from this site. Attested copy of the following documents are to be submitted along with the filled in application form. Forms can also be collected and submitted at the office of the Sub-divisional Officer (Civil), Ampari and Dadenggiri Civil Sub-division for the areas under sub-divisional jurisdiction.

Documents to be submitted along with the application form:

Must Submit:

  1. Two Passport Size Photographs
  2. Election Photo Identity Card (EPIC) of Self/Parent
  3. Certificate From Headman
  4. Land Patta/House Holding Tax/Nokma or Gaonbura Documents

Optional (At least 2 out of the list to be submitted):

  1. Parent's ST/SC Certificate
  2. Birth Certificate/SSLC Admit Card
  3. Ration Card
  4. NREGA Job Card
  5. Adoption Document/Affidavit
  6. Community Acceptance Certificate (mandatory in case of non-tribal parent)
  7. 2 passport size photographs

Apply for PRC?

Application form is to be collected and submitted at the Certificate Branch, DC Office. PRC is issued only for the purpose of Higher Education. Attested copies of the the following documents need to be submitted along with the application form.

  1. Two Passport Size Photographs
  2. Birth Certificate/SSLC Admit Card
  3. Pass Certificates/Mark Sheets from School/College
  4. Land Patta/Ration Card
  5. Certificate From Headman/ Nokma/Gaonbura
  6. Election Photo Identity Card (EPIC) of self, parents
  7. Caste Certificate of self, parents
  8. Supporting documents for course(s) applied for

Apply for Domicile?

Application form is to be collected and submitted at the Certificate Branch, DC Office. Domicile is issued only for the purpose of recruitment. Attested copies of the the following documents need to be submitted along with the application form.

  1. Two Passport Size Photographs
  2. Land Documents
  3. Birth Certificate
  4. Election Photo Identity Card (EPIC)
  5. School Certificates
  6. Certificate From Headman where necessary
  7. Supporting documents like Advertisement Copy etc.

Apply for Senior Citizen Certificate?

Application form is to be collected and submitted at the Certificate Branch, DC Office. Senior Citizen Card is issued to the Citizen who attained minimum age limit of 58 yea Rs. Attested copies of the the following documents need to be submitted along with the application form.

  1. Two Passport Size Photographs
  2. Birth Certificate/Medical Certificate as proof of age
  3. Election Photo Identity Card (EPIC)
  4. Pension Payment Order (for retired government servants)
  5. Residential Certificate/Proof
  6. Any other relevant document (please specify)

Apply for Income Certificate?

Collect application form from the Certificate Branch and submit along with documents like Salary Certificate or other proof of Income. Following documents to be submitted:

  1. Election Photo Identity Card (EPIC)
  2. Pension Payment Order (for retired government servants)
  3. Proof of requirement of Income Certificate
  4. Residential Certificate/Proof
  5. Salary Certificate from Employer
  6. Affidavit as Proof of Income (in case of self employed)
  7. Any other relevant document

Apply for permission for Special Event?

Application form is available at the Certificate Branch, DC Office. In case of Mela/Trade Fair etc. NOC are to be submitted from the following departments.

  1. NOC from DMHO (for Mela/Trade Fair)
  2. NOC from EE PWD (building) (for Mela/Trade Fair)
  3. NOC from Headman
  4. NOC from MeECL (for Mela/Trade Fair)
  5. NOC from SP Fire Service (for Mela/Trade Fair)
  6. Any other relevant document

Apply for Court orders for Birth & Death Registration?

Application form for Court Order under the Registration of Births and Deaths Act, 1969, for Birth/Death Registration should be collected from and submitted at the Judicial Branch, DC offce, Tura. The supporting documents submitted by the petitioner are:

  • An affidavit
  • An approach form
  • A certificate from Medical & Health Officer

Apply for Family Identity Card (Ration Card)?

Application form is available at the Supply Branch, DC Office, Tura. The following documents need to be submitted along with the filled in application:

  • Land Patta
  • Electors’ Photo Identity Card (EPIC)
  • Latest Receipt of Electricity Bill
  • Latest Receipt of House Holding Tax
  • Residential Certificate from Nokmas/ Ward Commissioners

Apply for Indo-Bangladesh Passport?

Application form can be collected from the DC Office, Tura and submitted in the same. Supporting documents need to be submitted are:

  • Land Patta/Nokma Document
  • Birth Certificate
  • SSLC Admit Card
  • 2 recent Passport Size Photograph
  • EPIC (Voter ID Card)

Society Registration?

Application form for the Society Registration should be collected from and submitted to the Establishment Branch, DC Office, Tura. Details to be furnished as follows:

  • The Constitution  of the Association
  • One copy of treasury challan showing the deposit of registration fee of Rs. 250/-
  • Meeting resolution of the Association

Marriage Registration?

Application form is available at the Registration Branch, DC Office.

Application form for Marriage Registration is available at the Establishment Branch, DC Office, Tura and should be submitted with the following necessary documents. Guidelines and Rules & Regulations for Registration under the Special Marriage Act 1954 are as follows:

  • The application for the Registration should be submitted 30 days before the intended marriage date for the unmarried couple / no application needed for the married couple.
  • During the time of marriage minimum of 3 witnesses should be present.
  • Documents to be submitted:
    • Birth Certificate should be submitted for the age proof.
    • Election ID card.
    • Marriage Proof in case of married couple (marriage certificate, wedding pictures etc).
