5.	Adoption proof	Court Order/ Registered Adoption Deed	Mandatory only in case of adopted child
6.	Supporting document	Self/Parent/Guardian EPIC Card
7.	ST Proof- Other State	ST Proof of either parent	Mandatory in case the parent have obtained ST proof from other State
